Week of October 18: imported notes keep both signatures

Notes written under supervision now arrive from SimplePractice with the provider's and the supervisor's signatures on their own lines.

Improved

  • Edit a claim on the CMS-1500 itself. A Prepared claim's CMS-1500 tab now has an Edit button, and the form's own boxes are typed in place: the member ID (1a), referring provider (17), diagnoses (21), Box 22 with the payer's reference number, prior authorization (23), every service line (24: date, place of service, CPT, modifiers, diagnosis pointers, rate, units) and accept assignment (27). Create Corrected Claim now lands straight on the copy's form in edit mode with Fixes to make pinned above it, and Edit in the Claims list opens the form too. The boxes that come from the client's record and your practice settings say where to change them. The small corrected-claim window is gone. Fixing claims.
  • Create Corrected Claim. Any claim the clearinghouse has taken, whether it is still in flight or already Paid, Denied, Deductible or Rejected, now has one Create Corrected Claim button. The original is archived (it stays on file with its last status and an Archived label, and drops out of Needs Attention), and a copy opens as a Prepared claim with the payer's reasons listed under Fixes to make and their claim number already in Box 22. Choose Original or Resubmission in Box 22 as the payer requires. For a claim sent to the wrong payer, the copy is billed to the client's current primary plan as an Original, so no void is needed, the way SimplePractice handles it. Fixing claims.
  • More care with an SSN used as your Tax ID. If your practice bills under your SSN and you haven't chosen where your Tax ID displays, it now prints on superbills only, not on invoices or statements. Tax ID fields hide an SSN until you click the eye icon, and Settings notes that an EIN keeps your SSN off client paperwork. Insurance setup.
  • Notes show the session's start and end times. At the top of a progress note, and on its printed or downloaded PDF, the session now reads like Oct 16, 2026 · 3:00 – 3:55 PM (55 min), taken from the appointment. Change the appointment's time on the calendar and the note follows, signed or not. On the PDF, the note's own date is now labeled Note date.
  • A session on a submitted claim keeps its date and time. Once a claim for a session has been sent to the payer, the calendar won't move it, by dragging or from the appointment window, so the record stays the same as what you billed. The appointment window says so at the top, and you can still edit everything else. To change the time, void or correct the claim first. Prepared claims that haven't been sent, and voided claims, don't lock anything. Fixing claims.
  • Choose how long before an idle account signs out. Under Settings → Practice Details → Security, a practice admin can now set Automatic Sign-Out for everyone in the practice, from 30 minutes to 72 hours. The default is now 24 hours (it was 72), so you stay signed in overnight but sign in again after a weekend or a day off. A minute before it happens, a message asks whether you're still there, and working in another tab counts as activity. Nobody is signed out during a session recording, including a paused one, or a video call. Security & HIPAA.
  • Edit an insurance payment one date at a time. When you edit a saved payment that you entered per session, each date of service has its own Client owes, Insurance paid and Write-off boxes, and only that session's invoice changes. Editing any manual insurance payment now starts from that payment's own client owes and write-off, not the totals of every payment on the claim. Recording a payment manually.
  • Enter insurance payments per session on claims that billed several sessions. On Add insurance payment, a claim that covers more than one session now shows one row per date of service instead of one row for the whole claim, so you can enter what the payer paid, the client owes and the write-off for each date from the EOB. Recording a payment manually.
  • Preferred names on the chart and the appointment details. A client with a Preferred Name is now titled by it at the top of their chart, with the legal name after it in gray, such as John Smith (Jonathan Smith). The appointment details panel shows the preferred name too, however you open it. Claims, invoices and notes still use the legal name. Client profile.
  • The chart sidebar is reordered, and every card collapses. The cards now run Client info, Client billing, Insurance, Upcoming Appointments, then Contacts. Each card has an arrow to collapse it, and a collapsed card stays collapsed on every client's chart until you open it again, still showing one line such as the balance or the insurer. Insurance now starts open. Client profile.
  • Delete a treatment plan. A treatment plan that isn't signed now has a Delete button on the plan page and a trash icon on its card in the client's chart. Deleting removes the plan, its goals and its comments, and cannot be undone. A signed plan is unlocked first; a plan waiting for your supervisor's co-signature can't be deleted. Treatment plans.
  • Statements show a running balance. Charges and payments now appear together in date order, with the balance after each line, starting from the balance on the statement's start date. You can see when a copay went unpaid and which payment followed it. Each charge shows its invoice number with the date and service of every session on it. Statements.
  • The credential mismatch warning shows how to fix it. When a payer's enrollment uses a different NPI than its claims, the enrollment details now explain the two ways out and have a Bill this payer under NPI … button. It opens that payer's billing settings with the enrolled NPI filled in, so only that payer's claims change. Payer enrollment.
  • Add the video link to your own reminder email. When you rewrite the Appointment Reminder under Settings → Client Notifications, a new Links menu offers Telehealth Join Link. It fills in the join link for telehealth appointments and stays blank for office appointments, so you no longer need to paste your video address into the template. Reminders.
  • Simpler past-visit counts on the import review cards. The Past visits row used to read "12 visits (12 attended, 0 canceled, 0 no-show)" on every card. It now reads "12 visits", and shows the split only when some visits weren't attended.
  • The payer's claim number fills in by itself. When a payer acknowledges a claim, their own claim number is now saved on the claim within a day, and again from the remittance. Void Claim, Edit & Resubmit Corrected and the secondary claim form start with it filled in, and if a claim has none yet, opening the form looks it up. The claim page shows the clearinghouse reference and the payer claim number side by side at the top, as the Claims tab shows the payer's number. Claim statuses.
  • Upload documents a payer asks for during enrollment. When a payer needs a PDF to finish an enrollment, such as a voided check, a bank letter or a signed form, you can now upload it from the enrollment's details under Settings → Payer Enrollment instead of contacting support. If the payer has a form, a Download button gets it for you. The file goes straight to the clearinghouse, and Practice Harbor doesn't keep a copy. Payer enrollment.

Fixed

  • Enrolling one payer under a different Tax ID no longer changes your practice's. In the enrollment wizard, a Tax ID entered for one enrollment, such as Medicare under your own SSN, used to replace your practice's Tax ID, so every other payer's claims would have used it. The wizard now keeps it with that enrollment only, and in a solo practice that payer's claims use the matching NPI and Tax ID. Billing profiles also record whether a Tax ID is an EIN or an SSN, so claims label it correctly, and a clinician billing under their own NPI is sent as a person rather than an organization. Payer enrollment.
  • An enrollment waiting on you no longer says "Rejected". When a payer needs a step from you, such as a document or an ID, the enrollment's details said Enrollment Rejected, with "No specific rejection reason provided by the payer" and a Resubmit Enrollment button. Nothing had been rejected. The details now open with the step to do, marked Action required in orange, and Resubmit appears only on enrollments the payer actually rejected. Payer enrollment.
  • Enrolling a payer with your individual NPI now bills that payer under it. In a solo practice that bills as the practice, a payer you enrolled for payment reports with your own NPI kept getting claims under the practice's NPI. The payer sends payment reports for the NPI on the claim, so they could fail to arrive, and the payer showed Credential Mismatch Detected. Enrolling with your own NPI now sets that payer's billing NPI to match, and its claims go out under your name and individual NPI, the way SimplePractice billing profiles do. Your other payers are unchanged. Payer enrollment.
  • Medicare enrollments ask for your PTAN. Medicare payers won't accept an enrollment without your Provider Transaction Access Number, and the enrollment wizard had nowhere to enter it, so the enrollment failed. For a Medicare payer, the Check for missing details step now has a PTAN field. Payer enrollment.
  • Edits to a duplicated consent form are kept. After you duplicated a consent form under Settings → Shareable Documents and edited the copy, the list kept showing the copy's old text, so opening it again showed the form as you had copied it, and saving from there undid your edit. Each form in the list now keeps its own changes.
  • Client owes from an insurance payment lands on the right invoice. When a claim billed several sessions, the client owes you entered for every date went onto one session's invoice. Each session's invoice now gets its own amount. Recording a payment manually.
  • Deleting a manual insurance payment clears it from the client's invoice. The deleted payment used to stay listed on the invoice, and the client owes it set wasn't adjusted. Delete now removes it from every invoice it touched and resets the client owes to what's still due, or tells you which invoice to check when the earlier amount isn't known.
  • Request client signature works from the chart again. On a client's chart, clicking Request client signature on a treatment plan opened the Share Documents wizard for a moment and then it vanished, so the signature request was never sent. The wizard now stays open through every step. Treatment plans.
  • Prescreener questions now appear on the contact form. Switching on Contact form under Settings → Prescreener saved, but the contact form kept showing only name, email, phone and message. The questions set to Contact form only or Both now appear on the form, required ones must be answered before the message is sent, and the answers come through in the notification email and the Inquiries panel and are saved to the client's record when you add them as a client. This works while online booking is paused too. Prescreener questions.
  • Supervised notes read cleanly after import. SimplePractice signs a supervised note in two columns, and the two signatures used to arrive run together at the bottom of the imported note: both names on one line, both licenses as one word, two dates and two IP addresses jammed together. The note now ends with two plain lines, Provider: name, license — signed date and Supervisor: name, license — signed date. Notes with a single signature are unchanged. Importing from SimplePractice.
  • Notes you never signed in SimplePractice arrive unsigned. A progress note left unsigned in SimplePractice used to come across as a signed note. It now arrives as an unsigned note you can finish and sign, the same as in SimplePractice. Signed notes and chart notes are unchanged. Importing from SimplePractice.
  • Diagnoses on older SimplePractice treatment plans now import. Plans written on SimplePractice's older layout put a Treatment Plan title above the Diagnosis section, and the import read the whole plan as free text and skipped the diagnoses listed in it, so a client whose only plans used that layout arrived with no diagnosis. Those diagnoses now come across with the plan. The plan's text is unchanged. Importing from SimplePractice.
  • Large imports no longer show "0 notes" on the review cards. On a practice with more than ten thousand notes, the cards that compare your SimplePractice export with the live chart could read 0 notes, 0 treatment plans for some clients although every note was on the chart. The cards now read the whole chart, however large the import.
  • Void Claim on processed claims. The Void Claim button only appeared while a claim was still in flight with the payer. Once the payer had answered, a claim marked Paid, Denied or Deductible had no way to be voided, although that is exactly when a wrong-payer or wrong-code claim needs retracting. Void Claim now appears on those claims too, and the help page spells out when it is offered. Fixing claims.
  • Insurance payments on Billing → Transactions now say who they're for. An ERA row used to read client Unknown with "ERA processed from Stedi transaction 68" as its description. It now names the client when the check is for one client, says 3 clients when it covers several, and reads No matching claims for a remittance on sessions billed from your previous EHR. The description names the payer: Summa Health Network · ERA, 2 claims.
  • Claims the payer turned away no longer sit at "Pending". A claim could read Pending for weeks after the payer or clearinghouse had already rejected it, because the automatic status check wasn't looking at newly filed claims. Every open claim is now checked against the clearinghouse's record within a day of filing and on every run after that, so a rejection shows as Rejected with the payer's reason and lands on Needs Attention without anyone clicking Check Status. A freshly filed claim now reads Submitted, then Accepted once the payer acknowledges it. Claim statuses.
  • Void Claim works on deductible and denied claims paid by a shared remittance. One insurance payment usually covers several claims, and voiding any claim on it stopped with "An insurance payment is posted against this claim. Delete or adjust that payment first", even when the payer paid $0 on that claim, as with a deductible or a denial. Deleting the payment would have unposted every other claim on it. The void now looks only at what the payer paid on the claim you're voiding: $0 lets the void go ahead and leaves the payment and its other claims alone. The Void Claim window also shows the client's and payer's names instead of "Unknown". Fixing claims.
  • Claims waiting on the payer no longer read "Partial". On a client's Billing tab, every claim that had been sent and was still waiting for the payer's answer showed an orange Partial badge, and its appointment billing page said Partial - Patient Owes, as if the payer had paid part of it. Nothing had been paid or denied. Those claims now show Pending on the Billing tab and Submitted to Insurance on the appointment billing page until the payer answers.
  • Remittance lines that match no claim are clearer, and stop nagging once reviewed. When a payer's remittance includes claims that weren't filed from Practice Harbor, such as claims from your previous system before you switched, the payment now lists them under Not matched to a claim in Practice Harbor while you edit it, not only when you view it, with what to do: allocate a line to its session if that session is scheduled here, otherwise leave it unallocated and set Payment Status to Received. The orange banner on the Insurance page now clears once a payment is marked Received or Not Received, the same rule the Needs Attention list already used, instead of staying up for money that correctly stays unallocated. Payments & ERAs.
  • A status check can no longer mark a claim Paid by mistake. When a payer's status line could not answer our inquiry at all (it returned an error about the request, with no claim status), the app read that reply as Paid, so a claim the payer was still adjudicating showed Paid with no payment behind it. Status checks now go by the payer's status category: an error about the request changes nothing on the claim, a finalized answer only reads Paid when the payer reports money, and the remittance decides the rest. Claim statuses.
  • Check Status gets real answers from payers. Insurance status checks were going out without the session's dates of service, without the client's gender, and, on a spouse's or parent's policy, without saying who the patient was, so payers could not find the claim and answered with an error. They now send what payers need, and when a payer lists several of the client's claims, the app picks the one for this session by date and amount. Claim statuses.
  • Telehealth claims no longer default to "telehealth outside the client's home." When a claim had no place of service of its own, a video session was sent as place of service 02 (client not at home), which payers can deny or pay at a lower rate for a session the client took from home. It is now sent as 10 (telehealth in the client's home). If nothing says the session was virtual, Edit & Resubmit Corrected now starts at 11 (office) instead of 02. Sessions with a location on the calendar are unaffected, since their place of service comes from the location.
  • Every location now shows its telehealth modifier, with None, 95 and GT to choose from. The modifier field used to appear only for video locations, so an office or a phone location billed a modifier you could not see — a phone location was fixed at audio-only 93, which most insurers do not pay for therapy. Add Location and Edit Location now show the field for every location type. It starts at 95 for telehealth and None for an office or phone, and the calendar follows your choice exactly: None means no modifier, even for telehealth. The audio-only and asynchronous codes (93, FQ, GQ) are no longer offered.
